Description

• This position involves providing compassionate nursing care to patients in their homes throughout the Bronx, NY area.
• The role is contract-based with weekday shifts and occasional weekend on-call duties.
• The nurse will perform clinical tasks such as wound care and medication administration, using generic technology systems for documentation.
  
  

Requirements

• Deliver clinical services during home visits, including wound care and medication administration.
• Provide nursing care aligned with individualized care plans and best practice standards.
• Monitor patient conditions and communicate updates to the care team.

Key Responsibilities

Qualifications

• Degree from an accredited nursing school (RN, BSN required or at least five years of experience).
• Minimum three years of clinical nursing experience, with 1-2 years in home care preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication skills.
