Title: Registered Nurse - Residential Housing

Description
- Work with mental health professionals to assist clients transitioning from hospitals to the community.
- Utilize the Critical Time Intervention model to ensure clients are linked with community providers.
- Diagnose and treat human responses to health problems through case management and counseling.
- Provide supportive or restorative interventions and execute prescribed medical regimens.
- Limited on-call coverage required.
- Complete medical and communicable disease risk assessments within mandated time frames.
- Coordinate medication delivery and reception with pharmacy.
- Track and manage medications, including returns to pharmacy.
- Observe self-administration of medications for detoxification as per protocols.
- Refer clients for necessary medical services.
- Conduct urine screenings according to program policy.
- Provide health education and consultation to clients individually or in groups.
- Valid NYS RN license.
- Knowledge of chemical dependency as a primary diagnosis.
- Excellent written and verbal communication skills.
- Experience in assessment and case management.
- Proficiency with generic office software tools.
- Ability to provide crisis intervention with interpersonal skills and maturity.
- Bilingual (Spanish/English) speaking preferred.
