SUMMARY:
Plan, organize, direct, coordinate and evaluate the home care services provided by ElderONE. Responsible for the clinical operations of the program (including those provided directly and via contract) that are necessary to assure a safe, effective and efficient program. Key Responsibilities Include:
- Reviews and monitors complaints regarding home care services and ensures satisfactory problem resolution in a timely manner.
- As part of the Management Team, develops the benchmarks that illustrate the areas of importance for the program and ensures the development and implementation of appropriate plans and goals.
- Oversees the coordination of the provision of care by contract agencies.
- Develop team members through education and mentoring. Encourages development of critical thinking, creativity and management skills. Establish/Implements leadership and care delivery
STATUS: Full Time
LOCATION: Rochester, NY
DEPARTMENT: ElderONE
SCHEDULE: M-F Days
ATTRIBUTES:
- Bachelor’s Degree in Nursing (BSN) required, Master’s Degree preferred
- Minimum of 5 years management experience in nursing and/or multidisciplinary teams in a home care setting.
- 1 year experience with frail elderly population
- Registered NYS Professional Nurse License
RESPONSIBILITIES:
- Unit Management. Develop programs and strategies to monitor the quality of patient care and patient outcomes; promote collaboration with medical and administrative colleagues to establish unit and daily operation goals
- Staff Management. Identify staffing needs; collaborate with the Director to develop and implement recruitment plans; prepare, monitor and maintain salary and non-salary budgets; participate in professional development of staff
- Leadership. Demonstrated ability to influence others in both internal and external environments to promote optimal individual patient care and healthcare services to the public
